Your First Visit
In order to meet this goal, your first visit with us will take approximately 1-1/2 hours and will consist of:

• Full series of diagnostic X-rays.
• Perio charting.
• Hard tissue charting.
• Thorough oral examination.

To expedite your first visit, please download and complete the New Patient form.
After opening the form you may type your information directly into the designated fields. Once completed, you may print a copy and bring it during your office visit.

	Your Second Visit On your second visit, we will discuss the results of the initial exam and your personalized treatment plan. Your treatment plan will include the procedures we feel you need in order to improve and maintain your oral health, and cosmetic procedures for you to consider in order to provide you with the perfect smile. You will be provided the cost of all treatments up front and financing options available based on the services you choose.


	EMERGENCIES Any true emergencies are always accepted and worked into the schedule. We will always do our best to accommodate you in every way possible.
